Golf Tournament Form

Please fill out this form if you’d like to participate in the tournament, donate to the cause, or just want to gain more information. Limits are four (4) per team. This page does not allow you to pay for registration or donation/sponsorship purposes. After you submit, this form will be sent to this year’s Golf Tournament Coordinators, Zach Kelly and Matt Evans. They will be in contact as soon as possible regarding your submission.

Contributions or gifts to the Lambda Omicron chapter of Alpha Kappa Psi are not tax deductible as charitable contributions for Federal Income Tax purposes. However, they may be deductible under other provisions of the Internal Revenue Code.

The deadline for registration is March 15. The tournament is March 31 at Riverwood Golf Course in Clayton, NC.
